SPRAY FOAM GUIDES — PROJECT OBSERVATION RECORD Version 1.0 | September 14, 2026 | U.S. English https://sprayfoamguides.com/tools/project-binder/ Use this blank record for observations you can collect safely or obtain from the responsible professional. It is not an inspection, installation procedure, diagnosis, permission to enter a work zone, or approval of a product or building assembly. Do not cut, probe, remove material, or enter a restricted area to fill a blank. Write “not known” or “not measured” when that is the accurate answer. Nothing in this file is submitted automatically. Keep your completed copy private. 1. PROJECT AND CONTEXT Private project ID or nickname: Record prepared on: Person making the record and role (owner, installer, inspector, other): Work dates or approximate period: Building area and assembly: General region / climate context (no street address needed): Existing conditions and known limitations: Who can answer unresolved technical questions: 2. PRODUCT AND DOCUMENT IDENTITY Manufacturer and exact product / formulation: Package and dispenser or equipment type: Lot / batch reference (retain privately if appropriate): Label photo reference: SDS title, component, revision and source URL: TDS / installation instructions title, revision and source URL: Applicable evaluation / listing record, edition and conditions: Documents requested but not received: Conflicts or substitutions and the source of written clarification: 3. WHAT WAS OBSERVED Make a separate entry for each location, date and condition. Location on sketch (for example, north wall bay N4): Observation date and stage (before work / before covering / completion / later): Who observed it; firsthand observation or another person's statement: What was actually seen or recorded: Measurement, units, method and instrument, if applicable: Who made the measurement and the scope of their role: Conditions and access limits affecting the observation: Photo / document reference: What the observation cannot establish: Open question and responsible follow-up person: 4. QUANTITIES AND CLAIMS Keep planned geometry, a supplier's stated yield, purchased quantity and measured installation results separate. A can count or attractive photo does not measure installed thickness, R-value, chemical cure, or code compliance. Planned dimensions / area / thickness with units: Source and conditions of any supplier claim: What was purchased: What was independently measured, by whom, with what method: Unknowns or reasons the numbers cannot be compared: 5. BEFORE, AFTER AND LATER FOLLOW-UP Record the same identifiable location; retain unedited originals privately. Before-work reference and date: Completion reference and date: Later follow-up date and elapsed time: Conditions that changed between records: Reported concern, action taken by the responsible party and outcome: Unresolved items (do not mark resolved merely because a photograph looks better): Inspection or specialist report and its actual scope, if available: 6. IF YOU CHOOSE TO SHARE A COPY Remove street addresses, contact details, faces, license plates, access codes and other identifying information unless the people concerned have agreed to its use. Review image location metadata. Retain the private original separately. Separate your own observations from contractor statements and expert findings. Disclose employment, free products, discounts, payment or other relevant ties. Describe any image edits; do not present generated or reconstructed images as photographs of the work. Do not send someone else's photos or reports as your own. This worksheet does not grant publication rights. Any publication needs a separate agreement on the material, credit, edits, translations and permitted uses. Do not include health records or private dispute correspondence in a public submission.
